By registering in this application, you will automatically gain access to the Trust Trust Branch. With the application, you always have practical functions at hand for solving your health and paying insurance premiums. After logging in to the application, you have: - the Insured's Trust card and your children's cards, - European card (EHIC) and EHIC cards for your children, - offline insurance card - in case you do not have access to the Internet - diagnose using self-diagnosis * - EU digital covid card ** - electronic health card - recipes on mobile - adding children - the possibility to apply for the Healthy Teeth benefit - Possibility to overpay for co-payments for medicines - SOS first aid card - information on health insurance payments - the possibility of paying insurance premiums * Self-diagnosis does not replace a qualified medical examination and is used exclusively for information and educational purposes. ** Trust makes available in the mobile application a digital covid EU card issued in the Slovak Republic, which is used to: - withdrawal of the digital version of the EU covid card (confirmation of vaccination / first dose, second dose, confirmation of the test performed, confirmation of overcoming covid-19), - storage of the digital version of the covid of the EU card in the client's facility - display of a digital version of the certificate to verify its authenticity at the place where services are provided that require off-line or on-line verification of EU licenses (to the extent and in accordance with the applicable methodology issued by the Public Health Office and the Ministry of Health) SR). There is confidence in making digital covers of EU licenses available to operators in fulfilling the obligation imposed by law (§ 15 par. 1 letter ai) of Act no. 581/2004 Coll. on Health Insurance Companies, Supervision of Health Care and on Amendments to Certain Acts as Amended). When making digital covid cards available, the EU does not collect any personal data from clients. The National Health Information Center (NCZI), with its registered office at Lazaretská 26, 811 09 Bratislava, is responsible for the accuracy of the data and thus for the correct provision of EU digital licenses in accordance with the Integration Plan Agreement concluded with the Trust (https://www.crz.gov .sk / data / att / 2929847.pdf). Personal data is stored only on the client's mobile device. No third party has access to them, not even Trust as the operator of the mobile application. Clients' personal data from digital covid EU cards are not disclosed or stored. If you decide not to use the Client with the MoA, after uninstalling the mobile application, the uploaded personal data from the digital covid of EU cards will be deleted. If the client believes that the personal data on the certificate is incorrect or out of date, he has the right to correct it, which can be applied at the National Center for Health Information (NCZI), with its registered office at Lazaretská 26, 811 09 Bratislava 1, e-mail: nczisk @ nczisk.sk.

Mobilná aplikácia Dôvera je zadarmo pre všetkých našich klientov. Registráciou v tejto aplikácii automaticky získate prístup aj do Elektronickej pobočky Dôvery. S aplikáciou máte stále po ruke praktické funkcie pre riešenie svojho zdravia aj platenia poistného. Po prihlásení do aplikácie máte k dispozícii: - preukaz poistenca Dôvery a preukazy vašich detí, - Európsky preukaz (EHIC) a EHIC preukazy vašich detí, - offline preukaz poistenca – v prípade, že nemáte prístup k internetu - zistiť diagnózu pomocou samodiagnostiky* - digitálny covid preukaz EÚ** - elektronickú zdravotnú kartu - recepty v mobile - pridanie detí - možnosť podať žiadosť o benefit Zdravé zuby - možnosť preplatiť si doplatky za lieky - SOS kartu prvej pomoci - informácie o platbách zdravotného poistenia - možnosť úhrady poistného * Samodiagnostika nenahrádza kvalifikované lekárske vyšetrenie a slúži výlučne na informačné a vzdelávacie účely. **Dôvera sprístupňuje v mobilnej aplikacii digitálny covid preukaz EÚ vydaný v Slovenskej republike, ktorý slúži na: - stiahnutie digitálnej verzie covid preukazu EÚ (potvrdenie o očkovaní/prvá dávka, druhá dávka, potvrdenie o vykonanom teste, potvrdenie o prekonaní ochorenia covid-19), - uloženie digitálnej verzie covid preukazu EÚ v zariadení klienta - zobrazenie digitálnej verzie potvrdenia na overenie jeho pravosti na mieste, kde dochádza k poskytovaniu služieb, ktoré si vyžadujú off-line alebo aj on-line verifikovanie covid preukazov EÚ (v rozsahu a v súlade s platnou metodikou vydanou Úradom verejného zdravotníctva SR a Ministerstvom zdravotníctva SR). Dôvera je pri sprístupňovaní digitálnych covid preukazov EÚ prevádzkovateľom pri plnení zákonom uloženej povinnosti (§ 15 ods. 1 písm. ai) zákona č. 581/2004 Z. z. o zdravotných poisťovniach, dohľade nad zdravotnou starostlivosťou a o zmene a doplnení niektorých zákonov v znení neskorších predpisov). Pri sprístupňovaní digitálnych covid preukazov EÚ nezískava od klientov žiadne osobné údaje. Za správnosť údajov a tým aj správnosť poskytnutia digitálnych covid preukazov EÚ je zodpovedné Národné centrum zdravotníckych informácií (NCZI), so sídlom Lazaretská 26, 811 09 Bratislava v súlade s Dohodou o integračnom zámere uzavretou s Dôverou (https://www.crz.gov.sk/data/att/2929847.pdf). Osobné údaje sa ukladajú iba v mobilnom zariadení klienta. Nemá k nim prístup žiadna tretia strana, dokonca ani Dôvera ako prevádzkovateľ mobilnej aplikácie. Osobné údaje Klientov z digitálnych covid preukazov EÚ sa nezverejňujú a neuchovávajú. Ak sa Klient rozhodnete MoA ďalej nepoužívať, po odinštalovaní mobilnej aplikácie sa nahraté osobné údaje z digitálnych covid preukazov EÚ vymažú. Ak sa klient domnieva, že osobné údaje na certifikáte nie sú správne alebo sú neaktuálne, má právo na opravu, ktorú si môže uplatniť v Národnom centre zdravotníckych informácií (NCZI), so sídlom Lazaretská 26, 811 09 Bratislava 1, e-mail: [email protected].
