Altus Care is a platform for individually tailored digital care management allowing two-way communication between healthcare providers and patients to increase adherence & patient outcomes.
The platform combines the ability to deliver expert knowledge with smart technology, and allows providers to create individual plans for treatment, aftercare and rehab exercises.
Patients are able to see a daily simple task list, report symptoms, chat with healthcare providers and receive intelligent notifications that helps the patient stay motivated to follow the provided plan. Healthcare providers can follow and monitor all their patients in a dashboard, where patients that need urgent attention and at risk patients are highlighted including individual patient statistics.
The platform enables patients to get personalized care plans and healthcare providers to maximize their time and efficiency based on patient risks.
To be able to use the Altus Care app, your healthcare provider needs to assign you a treatment plan. Without this, the app can't be activated.
As a healthcare provider, you need to contact the Altus Care team to access the web platform that allows for development of treatment plans.
