Description
This is an app for Pinnacle’s Health Management program which provides personalized care management and wellness services to the employees and their families. This offers a simple way to transmit health measurements from:
• Glucose Monitor
• Blood Pressure Monitor
• Weight Scale
• Ear Thermometer
• Finger Pulse Oximeter
To a HIPAA compliant health cloud which allows members of the patient’s healthcare team, family and friends to receive unrestricted, unlimited access to historical diagnostic test results, as well as elect to receive event notifications based on your care team digital care plan prescribed for you. You have full control to review complete and accurate test results in a graphical form by Day, Week and Month in an effort to educate you and everyone involved in managing your health care goals. Whether it is transmitting data, checking your messages from your care team, or participating in a virtual visit, the PHM ecosystem simplifies your care for everyone involved.
