Description
The HPC Ontario app helps users find Ontario facilities offering 24-hour care for patients facing a life-limiting illness. Users can search the interactive map by city, address, or postal code, or by turning on the GPS function to get a list of Inpatient Hospice Palliative Care facilities in the area. Users can then select a facility to find contact information, directions, admission criteria, and available supports.
The Inpatient Hospice Palliative Care Directory was designed by The Princess Margaret Cancer Centre to empower patients, families and healthcare providers in making timely and informed choices about end of life care. This app is meant to help families and healthcare providers access timely and accurate information on hospice palliative care services across Ontario.
